Should chrome tile trim be flush with the tiles, or have a slight lip? Or is a matter of preference?

Flush, if it's set back (as yours is), then the trim is too deep. Generally speaking, the trim would be 1-2mm deeper than the thickness of the tile to accommodate the tile and adhesive.

Thank you everyone. I am only a DIY amateur, I find it hard to challenge a bathroom installation company as I am less qualified. But you have backed up what I believed to be the case, that tiles should be flush with the trim.
