    I know these are automated feeds but after reading the story someone must have known that a concrete slab propped up with supports would fall when the support was taken out! Poor supervision and training is usually the culprit.

    I once watched a 'builder' taking accros out after inserting a 50mm piece of timber above a door that had bricks up to the eaves, he couldn't understand why the door was stuck until we pointed out his bowed bricks/timber. The whole wall had to be re-built, what a numpty he was.
