Default Verona Crema Porcelain 600 x600mm

These tiles where laid about 6 months ago. Within a month one section of tile lifted, the grout Bal Rapid Flexible adhesive was still stuck to the plywood. We had these tiles replaced.
It has now started in another part of the room and at the bottom of the stiars in tiles 300x300.
the tiler said it is caused by the plywood being wet
    Question
    1. Am i correct to assume that he should have advised us of this at the time and not laid the tiles?
    2. Is there any other reasons for tiles not bonding to the adhesive ( if it is to thick, wrong type) etc?
    3. Any thing else I should consider?
    Thank you

How thick is the ply and how was it fixed.

Was the plywood stored outside then laid wet.

    What adhesive was used

The tiles where laid on 19mm WBP screw fixed to timber battons on a concrete floor.

The WBP was dry when laid but did get wet a one point during construction

Bal Rapid Flexible adhesive was used.

    thanks
