I'd be thinking of using a decoupling membrane after repairing too!

Could use a low viscosity epoxy repair compound like PSRS from Ardex. This soaks into the surface and displaces the cement as the binding agent to become an epoxy system. I not used it on old concrete but have on screeds both cement based and anhydrite and it works a treat. No reason to think it wont work on concrete.

Good point whitebeam. If it is screed the PSRS is fab stuff. Not cheap but does a really good job of knitting the surface back together. Have come across one or two other systems as well. would be able to self level straight onto then. No need for uncoupling - although that might be slightly cheaper option - not sure.
